Grafton VT starting new community Nurse program

​Now Seeking: Independent Grafton Community Nurse
Request for Proposal (RFP): To provide approximately 40 hours per month of non-
emergency community nursing services to residents of Grafton. This is a
community-based, home-visiting role focused on helping people remain safely at
home, navigate healthcare, and connect with needed services.

Preferred Qualifications

 Vermont-licensed RN or APRN
 BSN or higher
 Minimum 3 years’ experience in elder care, home health, hospice, or
community/public health nursing
 Strong health assessment, teaching, and communication skills
- Experience with case management and community resource
navigation
- Familiarity with electronic documentation (training on
FLOSSIE/REDCap provided)
- Demonstrated professional maturity and ability to work independently
- Excellent time management and organizational skills
- Reliable car (mileage reimbursement provided)
- Independent professional nursing liability insurance (coverage
requirements to be defined in final agreement)
