Kristin Barnum
Executive Director

Kristin Barnum, RN, BSN, MBA, is a seasoned Healthcare Executive and Clinical Leader with over 30 years of experience dedicated to advancing community health, hospice, and palliative care. Throughout her career spanning for-profit, non-profit, and governmental organizations, Kristin has focused on breaking down operational silos to create integrated, patient-centered care models that allow individuals to remain safely in their homes.
Kristin began her nursing career in oncology at Emerson Hospital in Concord, MA. While completing her BSN at Rivier College, she completed a specialized hospice internship with Home Health and Hospice in Nashua, NH, contributing to the launch of the Community Hospice House in Merrimack, NH. She then joined Home Health Foundation in Lawrence, MA as a Clinical Manager and Educator. There, she co-created an innovative preceptor program to recruit new nursing graduates into community health and developed cultural diversity training initiatives that she presented at both state and national conferences.
Her leadership trajectory continued as Hospice Director for Nashoba Valley Nursing Service in Shirley, MA. At Nashoba, Kristin led the implementation of the agency’s first Electronic Medical Record (EMR) system, sparking a passion for leveraging data-driven quality improvement that motivated her to earn her Master of Business Administration (MBA).
After relocating to the Upper Valley in 2007, Kristin was recruited in 2009 by BAYADA Home Health Care to build and launch their ground-up hospice program. Under her leadership, she scaled the startup into a high-performing regional division generating over $5 million in revenue while achieving a 100% deficiency-free initial CHAP accreditation.
Currently, Kristin serves as Executive Director of Community Nurse Connection (since 2021)
